Terms & Conditions - Singing Lessons 

 

Privacy:

 

In Good Voice needs to collect information about you for the primary purpose of providing you with a quality service. This information will be used for:

  1. The administration purpose of running the practice;

  2. Billing either directly or through an insurer or compensation agency;

  3. Use within the practice for your ongoing management.

 

You can request to view Information about your assessment results and progress. This request needs to be received by In Good Voice in written form. We will do our best to comply with your request within 14 days’ notice.

 

In Good Voice is committed to ensuring your privacy both in your session and through the safekeeping of all client documentation. Your personal details and information will not be disclosed to anyone operating outside In Good Voice. If, for any reason this is required, In Good Voice will obtain additional written consent from you.

 

Accounts:

 

Your contact details may also be used to assist us with fee collection or passed on to debt collectors in the event that your account remains unpaid beyond a reasonable time. In this event, collection costs will also be passed on to you.  Payment is to be made on the day by cheque, cash, or card. We also accept direct deposit - our banking details are available on request. 

 

Cancellation notification is required 24 hours prior to the appointment.  A cancellation fee at the cost of $50 applies for all appointments if this notice is not given. If a student does not turn up for the lesson without notice, the full amount for that lesson will be payable.

Agreement:

Before commencing singing lessons, you must agree to the following:

  • I have read the information above and understand the reasons for collecting the information and the ways in which the information may be used.

  • I understand that it is my choice as to what information I provide and that withholding or falsifying information might act against the best interests of my assessment and progress.

  • I am aware that I can access my personal and treatment information on request and if necessary that if, in exceptional circumstances, access is denied for legitimate purposes, that the reasons for this and possible remedies will be made available to me.

  • I understand that the practice must obtain additional consent if the information collected is to be used in any ways other than that outlined above. 

  • I consent to the handling of my information by this practice for the purposes set out above, and take ultimate responsibility for the payment of my account.
